Consent to sharing data with partners

By ticking this box in the form, I give my consent to PT Konsultasi Kelimpahan Bali (Indonesia, NIB 2202230073918, address: Jalan Raya Sanggingan No. 75, Kedewatan, Ubud, Gianyar, Bali 80571, Indonesia), hereinafter the “Operator”, to share my personal data with partners involved in organising my programme.

Recipients

clinics and specialists in South Korea — for consultations and treatment selection;
interpreters;
transport companies and hotels;
services used to handle requests: Vercel (website hosting), Google (request spreadsheet), Telegram (request notifications).

Cross-border transfer

I understand that some recipients are located outside my country of residence, including in South Korea and Indonesia, and I agree to such transfer to the extent needed to organise the programme.

What data

Name, age, contact details, country, chosen programme and travel dates. Medical data is shared only with separate consent.

Duration and withdrawal

This consent is valid until the programme has been organised, but no longer than 3 years, and may be withdrawn by writing to hello@antiagingconcierge.com.
